Irosha Vaderi is a 68 year old lady with metastatic breast cancer. Irosha was diagnosed with breast cancer 9 years ago and following treatment was considered in remission. Eight months ago Irosha sought advice from her General Practitioner following appearances of lumps near her mastectomy site. She reported shortness of breath and longstanding dry cough. Following a Positron Emission Tomography (PET) it was confirmed that the cancer has metastasized to her lungs. After several appointments with her oncologist an informed decision was made between the health care professionals, Irosha and her family that the treatment would continue as palliative care only.
Irosha has a subcutaneous cannula and syringe driver in situ, delivering morphine and metoclopramide. She is ordered oral Ordine liquid prn for breakthrough pain relief. Irosha is from Sri Lanka lives with her husband, son and daughter-in-law and four grandchildren. Her son is not coping with the Advance Care Directive. Irosha has a fungating chest wound with malodorous exudate. This wound requires dressing every few hours.

Introduction
Care plans in nursing are involved in provision of a direction that individualised care of a patient. Care plans are based on the unique diagnosis of the patient and is designed according to the individualised needs of the patients (Gulanick & Myers, 2016). The case study depicts the condition of Irosha Vaderi suffering from metastatic breast cancer.
Body
The nursing problems in terms of physiology was identified as respiratory problem. In terms of behavioural problem it was related to problems of coping. While in terms of functional the problem was with safety.
I will try to implement indirect nursing interventions along with direct nursing interventions in addition to proper communication, over the next 12 months, to appropriately identify the severity of cancer in the patient and will initiate treatment. Every 3 to 6 months I will see the patient and will make sure to use proper interventions to access the control of environment and accordingly adjust therapy. I will try to attend educational intervention programs and apply those learning in the care plan.
The indirect nursing treatment would involve patient encouragement to attend therapies, care environment improvement, supporting patient’s partner/family members and self-awareness of nurses. Direct interventions would involve the informational consultant and psychological and emotional support along with communication competence (Hershman et al., 2013). The inventions would help to reduce postoperative anxiety and emotional distress in the patients.
Conclusion
Through nursing care plans, the nursing priorities are identified which helps to attain a SMART goal for the nurses. Additionally interventions are suggested that are required for reducing emotional distress and postoperative anxiety in patients
