Rationale:

There are two reports for the Internship, one completed midway through the placement and one at the conclusion. Both use the same criteria so that students can identify any changes in their performance between the two reports.

The completion of the Internship Mid-Placement Report midway through the student placement provides opportunity to consolidate comments on student progress and to highlight the areas still to be addressed. General comments may be included to respond to specific skills if the Workplace Mentor so desires.

The Mid-Placement Report will address the following Key Performance Indicators as measured by the Workplace Mentor:

Teamwork
 Collaboration
 Problem Solving
 Decision making
 Awareness
 Motivation

Note on Assessment Value:

Students assessed with an ‘Unsatisfactory’ rating in the Mid-Placement Report will require additional consultation and development in order to proceed with and complete the internship.
A development program will be designed and provided to the student if an ‘Unsatisfactory’ rating is allocated in the Mid-Placement Report
