Question:
Discuss about the Review Of Integrated Assessment Health Practices.

Answer:

Nursing Assignment
The video provides a review of integrated assessment health practices. The assessment is carried by a nurse practitioner whose responsibility is to examine the client based on the holistic practices. The examination is done after a successful interaction between the patient and the nurse. From the video, the nurse strives to establish to enhance the bond so that she can be able to acquire the necessary information which will enable him to perform the task.
The selected NMBA registered nurse standard for practice is based on the comprehensive assessment (Theresa, 2018). The practice describes facts on the client’s medical, physical and psychosocial condition of the client. It also addresses the patient needs and provides a detailed analysis of the patient’s record based on the continuous, comprehensive assessment.
Based on the video, the activity is being undertaken in an environment which allows open communication between the nurse and the patient. The nurse undertakes the examination task while observing the patient’s feelings and the manner in which he explains his condition. The video also portrays the nurse as one who knows how to uphold work ethics because he uses polite language whenever seeking any information from the patient.
According to Khushminder (2017), one of the advantages of holistic practice is that it establishes a proper bond between the nurse and the patient. This can be evidenced through the way in which the nurse tries to express himself to the patient during their first encounter. When the nurse gets to the patient, he tries to introduce himself in a manner which makes the patient to feel the essence of friendship. Although the patient appears to be a bit nervous during the initial stages of interaction, the mode of communication which the nurse uses makes friendship to develop, hence leading to open communication regarding the health issues affecting the patient.
After conducting some tests, mixed reaction seems to arise based on the patient’s response and physical appearance. The nurse is also using professional tactics to make the patient remain relaxed while the examination is being done. There is also high utilization of ethical language while telling the patient concerning the outcome of the test and this is making him to remain happy. Some of the words that makes him to have positive reactions towards the test include, “please, thank you and so forth.”

The good thing about the experience is that the patient was able to speak out what he was feeling. The patient is being examined by the nurse based on the feeling that’s why the nurse prefers to test for fever at first and after that proceed for another physical check-up. The good thing about the experience is the framed questions which seem to follow a particular sequence thus motivating the patients to gain more confidence when providing answers. The best part of it is that the patient was satisfied with the results and also the nurse provides a room for further consultation in case the patient may need him afterward.
The analysis of the health assessment can be done by first obtaining the biographical data of the patient (Diamond, 2015). This data may include the name of the client, the address, and age of the client, the current occupation, health care financing and also obtaining the most preferred medical Centre if any.
Another area of analysis can be based on the chief complaint or the reason for the visit. This analysis helps in determining the reason for the client visit which is expressed by the client himself. According to Sandra (2011), the outcome of the clinical assessment depends on the approaches that the nurse chooses to adapt to ensure the patient cooperate in the exercise. Therefore, it is fundamental for the nurse to ensure he uses the best tactics that can increase the patient’s confidence while responding to the clinical questions.
Another factor can be based on the history of present illness (Scott & Scott, 2010). This area of analysis helps to determine the onset of the symptoms, the time when first symptoms occurred, whether there is frequent occurrence of the symptom or the symptom has developed suddenly. Additionally, the nurse can base his analysis on the activities which might have led to the development of the problem if any.
Another area of analysis can be based on the patient’s history. This technique helps to track down the record of the client based on the history (Christiane, 2012). This can be done through obtaining information concerning the past illnesses, allergies to drugs, environmental agents and the medications which he might be using during the time of analysis or in the past few days or weeks.
Another component of the analysis can be based on the family history of illness. According to Kathy (2013), identifying family history helps a nurse to find out if the disease has inheritance basis and if the patient is at the risk of developing the illness in the future. The nurse can also base his analysis on other factors that contribute to illnesses such as personal habits, hobbies and other factors associated with lifestyle.
To successfully conduct and manage assessments, it is fundamental for the medical practitioners to relate theory and practice (Jane, 2015). There is also need to develop a planned procedure which can be used to enhance the level of interaction between the nurse and the client. Conducting extensive research and adapting to modern technology is also a good strategy which can make one conduct excellent clinical assessments
Additionally, the nurse should ensure that he works toward satisfying the interest of the patient and also according to the professional ethics. According to Cheri (2011), this strategy plays a key role in making the patient to be confident when giving the required details. There is also the need to employ the value of specialized knowledge in narrowing the gap, offering institutions evidence for planning, and structuring the organization of care.
For nurses to foster local adaptation of the best and recommendable practices, it is fundamental to understand the importance of knowing emerging issues concerning healthcare and other key factors that determine the quality of care such as technology and innovation (Karen, 2010). Nurses should also understand the importance of interdisciplinary in their profession and some of the issues that influence their interaction with patients such as communication and interpersonal skills.
