Answer:
Introduction
I was able to demonstrate a high level of interpersonal, verbal and written communication skills while working with Aboriginal and Torres Strait Islander communities. One situation where I demonstrated the skills effectively was while educating the youth population as a part of a Well Being workshop.
The task involved educating the youth (aged 16 to 25 years) on healthy lifestyle which can prevent the risks of chronic conditions like Obesity, Diabetes and Hypertension, and to increase awareness on the adverse health effects of addition (to smoking and alcohol consumption).
To do this, I had to educate the youth on the importance of physical exercise, healthy diet, through educative classes, workshops, educative materials. Lesson plans, presentations and handouts were designed for education and important contact numbers for help was also shared in the process (Schmidt & Hunter 2013).
At the end of the workshop, the participants were able to understand how sedentary lifestyle and unhealthy diet (as well as smoking and alcoholism) increased risks of Obesity, Cardiovascular Diseases, Diabetes, Hypertension as well as related diseases, and they were able to identify healthy diet and lifestyle choices that can reduce the risks.
Demonstrated clinical knowledge and clinical problem solving abilities.
Situation:
A situation where I was able to demonstrate my clinical knowledge and clinical problem solving abilities was while working with elderly individuals from the Aboriginal and Torres Strait Islander communities.
Task:
The task at hand was to assess the needs of the patients in aged care centers, trying to understand their care needs and also to assess the environmental risks or hazards that can disrupt their health and well being.
Action:
The actions I took included assessments of nutritional status, functional status/ability, motor coordination and mental health status for the assessment of the residents. To assess the environmental risks, I had to identify objects in the environment that can increase the risks of accidental falls and injuries.
Result:
At the end of the assessment, I was able to assess the health status and care needs of the residents, which allowed the design of personalized care plan for each resident, and also helped to reduce the risks of accidental injuries, as a result of which the rate of accidents reduced in the facility.

Demonstrated ability to work within a team.
Situation:
A situation where I was able to demonstrate my ability to work within a team was while working within an interdisciplinary team in the hospitals and healthcare units.
Task:
Working within an interdisciplinary team consisting of professionals from different dimensions of healthcare and social support, I had to often helped in the development of communication between the various professionals, sharing important findings and identifying the roles of each professional and following up with them regularly about the progress.
Action:
The actions that I took to ensure effective functioning of the team was to keep accurate case notes and case histories, and identifying what information needs to be shared with whom. Also I had to ensure that the records are updated on a timely manner.
Result:
The result of being a part of a multidisciplinary team was that I developed significant leadership skills and the ability to communicate with diverse healthcare professionals, understanding their terminologies and sharing them with relevant care professionals.

An understanding of the professional, ethical and legal requirements of the Registered Nurse or Midwife.
Situation:
I was able to gain an insight and understanding of the various professional, ethical and legal requirements that should be followed by Registered Nurses and Midwifes during my education on the Nursing profession.
Task:
With the completion of the curriculum, I had to engage in various hands on activities which involved active research and helping patients. The various assignments that I needed to complete required a comprehensive understanding of the ethical, professional and legal requirements in this profession.
Action:
I had to prepare presentations and reports, for the completion of which I had to study the codes of practice outlined by the governing bodies. Also preparing for examinations helped strengthen the knowledge.
Result:
The end result was a significant development of my understanding of the codes of practice and professional requirements that are in accordance with the ethical, professional and legal standards of practice.

A demonstrated understanding of the role of the Registered Nurse or Midwife in applying continuous improvement and quality and safety
Situation:
While working with elderly patients, I was able to demonstrate plans for continuous improvement in safety and quality through the process of reflection, self evaluation, and literature research on best practices in continually improve the practice standards in terms of quality and safety.
Task:
The task of continuous improvement required reflecting back upon my own actions and thoughts, evaluating those on the merits of their outcomes and effects and constantly trying to identify the best known clinical practices through literature reviews and analysis.
Action:
The key actions involved in the process were related to secondary literature research, evaluation of the merits of the research paper to identify the best practice and integrating them to by professional practice.
Result:
The end result was that I was able to identify several key strategies that can be used to improve the well being of the elderly patients, one such strategy was to reduce the risks of fall related injury, for which interventions to improve the gait and balance was an important consideration.

A demonstrated understanding of NSW Health’s CORE Values – Collaboration, Openness, Respect and Empowerment
Situation:
Working with Aboriginal anfd Torres Strait Island communities, I was able to demonstrate the core values of NSW Health, such as collaboration (with multi disciplinary team), openness (in communication with the indigenous population), respect (for their culture and customs) and empowerment (of these communities)
Task:
Helping the Aboriginal and Torres Strait Islander communities, I had to engage in active communication with the people as well as a multidisciplinary healthcare team. I also had to educate the youth on healthy lifestyle and adverse effects of addiction.
Action:
I conducted workshops to educate the youth on healthy lifestyle and health risks, and also worked with the elderly in the assessment of their health. Working with them I had to show cultural sensitivity as well as openness/honesty in my own views and through education I had the opportunity to empower them.
Result:
As a result of these actions I was able to develop a good relation and trust with the communities I worked with, and was able to develop a therapeutic relation with the service users.
